Care Coordinator

Job Description

As a Biologic Care Coordinator at Specialty Rx Health Solutions your primary responsibility would be to coordinate the care and treatment of patients with complex medical conditions/ who require specialty medication. You would work closely with patients, physicians, pharmacists, and other healthcare professionals to ensure that patients receive high-quality, cost-effective care that is tailored to their individual needs.

Your specific job duties may include:

Patient Support and Advocacy:

· Provide support and guidance to patients throughout their healthcare journey.

· Advocate for patients' needs, ensuring access to resources, services, and therapies.

Coordination of Care:

· Collaborate with SRx health, physicians, pharmacists, and other healthcare providers to ensure that patients receive appropriate medication and therapy.

· Assist patients and physicians with insurance paperwork, applications, and appeals related to drug access and reimbursement.

· Develop and implement individualized care plans in collaboration with the healthcare team.

· Coordinate appointments, tests, and consultations for timely scheduling and follow-up.

Education and Support:

· Providing education and support to patients and their families to help them better understand their medical conditions, treatment options, and self-care strategies.

  • Educate patients on medical conditions, treatment options, and self-care strategies.
  • Empower patients to actively participate in their healthcare decisions.

Documentation:

· Maintain accurate patient records and document interactions, interventions, and outcomes.

· Generate reports and communicate patient progress to the healthcare team as required.

Preparation:

· Preparing medications for administration by the physician, prepping patient for on-site injections for therapy and assisting with medication administration

Compliance and Quality Assurance:

· Ensuring that all care provided meets regulatory and quality assurance standards, including compliance with HIPAA, Medicare, and Medicaid regulations.

Communication:

· Maintaining open communication with the company SRx, as well as, patients, caregivers, physicians, and other healthcare providers to ensure that all stakeholders are informed and involved in the care process.

Data Analysis and Reporting:

· Analyzing data and generating reports to evaluate the effectiveness of care plans and identify areas for improvement.

Corporate Duties:

· Maintain confidentiality of client and corporate information and discuss the same with appropriate SRx Health Solutions’ personnel.

· Adhere to SRx Health Solutions and manufacturer Policies and Procedures

· Ensure mandatory trainings are completed in a timely manner.

In addition to these core responsibilities you will also be responsible for:

  • Assist the prescribing physician to complete patient enrolments; including gathering all pertinent and appropriate information to complete the necessary forms (private and/or provincial insurance) in a concise, organized, and professional manner
  • Liaise and advocate with third-party providers and provincial governments as necessary to determine coverage options
  • Review patient charts (paper and electronic based)
  • Ensure patients are scheduled and notify physicians of any requests
  • Complete Welcome Call activities with the patient according to defined scripts
  • Facilitate any required communication with physicians, Patient Support Programs, or other health care professionals involved in the patient’s circle of care
  • Act as a central resource liaison for program stakeholders, which include physicians, nurses, patients, and clinics
  • Ensure patient records are maintained and data-accurate to meet program reporting
  • Assist physician with pre-clinical workup / biologic injection training
  • Biologic health teaching

Position is for Gastroenterology

Job Type: Full-time

Salary: $20.00-$25.00 per hour

Expected hours: 8 per week

Schedule:

  • 8 hour shift
  • Monday to Friday

Ability to commute/relocate:

  • Moncton, NB E1C 1B9: reliably commute or plan to relocate before starting work (required)

Education:

  • Secondary School (preferred)

Work Location: Hybrid remote in Moncton, NB E1C 1B9
